Southlake Orthopaedic Practice Hosts Carroll Independent School District Student Athletic Physicals

For the 2014-2015 school year, student athletes in the Carroll Independent School District will be completing their annual physical exam at All-Star Orthopaedics in Southlake.

Southlake, TX — The Carroll Independent School District (CISD) recently announced that All-Star Orthopaedics in Southlake will be dedicating specific time slots to student athletes to undergo physical examinations for the 2014-2015 school year. CISD requires any students who want to participate in sports, cheerleading, or drill team to have an updated physical examination annually.

Tickets for appointments at All-Star Orthopaedics are being sold on the CISD Athletics website for 30-minute time slots during the designated dates and times below:

• Saturday, May 3, 2014 from 9 am – 2 pm

• Wednesday, May 7, 2014 from 5 pm – 7:30 pm

The examinations will be conducted by sports medicine and orthopaedic specialists at All-Star Orthopaedics, including physicians, physical therapists, and nurses. Other local physicians will be joining the All-Star Orthopaedic staff to assist, as well. The examination times are being offered as a courtesy to parents and students, but students may have their examination completed by another physician, should it be more convenient. CISD will only accept examinations completed by a Physician, a Physician Assistant licensed by a State Board of Physician Assistant Examiners, a Registered Nurse recognized as an Advanced Practice Nurse by the Board of Nurse Examiners, or a Doctor of Chiropractic.
