Medical Request Forms
For your convenience, we offer a secure online method of requesting access to your medical records and imaging CDs containing the results of any diagnostic or screening tests performed. Medical records are typically processed and made available within 48 hours of receipt of your online order. For image requests, the average turnaround is closer to 2-3 days, excluding weekends and holidays.

In cases where you need our office to assist with filling out or processing forms—such as FMLA or Disability—please use the link below.
